【摘要】：目的通過多中心的社區(qū)干預(yù)試驗(yàn),評(píng)價(jià)社區(qū)高血壓疾病細(xì)節(jié)管理對(duì)高血壓患者相關(guān)知識(shí)知曉度的影響。方法在上海市4個(gè)中心城區(qū)的社區(qū)衛(wèi)生服務(wù)中心中共招募1 442例高血壓患者,分為細(xì)節(jié)管理組961例、日常管理組481例。細(xì)節(jié)管理組:設(shè)立社區(qū)健康管理專員專崗,增進(jìn)社區(qū)醫(yī)生與患者的溝通,提高患者服藥依從性,對(duì)患者進(jìn)行生活方式指導(dǎo),并為社區(qū)醫(yī)生定期收集血壓等相關(guān)信息并反饋;開發(fā)和應(yīng)用社區(qū)管理信息平臺(tái)和患者管理工具。日常管理組:采用常規(guī)的管理模式。結(jié)果細(xì)節(jié)管理組和日常管理組對(duì)高血壓相關(guān)的各項(xiàng)知識(shí)知曉情況均較基線有所提高。干預(yù)后細(xì)節(jié)管理組的收縮壓和舒張壓均低于日常管理組,血壓達(dá)標(biāo)值、高血壓患病危險(xiǎn)因素、高血壓防治原則等知曉度評(píng)分,細(xì)節(jié)管理組顯著優(yōu)于日常管理組,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。細(xì)節(jié)管理組收縮壓與知曉度評(píng)分總分的相關(guān)系數(shù)為-0.352(P=0.000),舒張壓與知曉度評(píng)分總分的相關(guān)系數(shù)為-0.262(P=0.000)。結(jié)論上海社區(qū)高血壓疾病細(xì)節(jié)管理是提高高血壓患者相關(guān)知識(shí)知曉度和血壓控制水平的有效措施。
[Abstract]:Objective to evaluate the influence of community hypertension detail management on the knowledge of hypertension patients by multi-center community intervention trial. Methods A total of 1 442 patients with hypertension were recruited from community health service centers in four central districts of Shanghai. They were divided into detail management group (961 cases) and daily management group (481 cases). Detail management group: set up community health management specialist post to improve communication between community doctors and patients, improve patients' compliance with medication, provide lifestyle guidance to patients, and collect blood pressure information and feedback regularly for community doctors; Develop and apply community management information platform and patient management tools. Daily management group: adopt the normal management mode. Results the knowledge about hypertension in detail management group and daily management group were better than baseline. After intervention, systolic and diastolic blood pressure in detail management group were lower than those in daily management group, and blood pressure reached the standard value, hypertension risk factors, hypertension prevention and treatment principles and other awareness scores. Detail management group was significantly better than daily management group. The difference was statistically significant (P 0.05). In detail management group, the correlation coefficient between systolic blood pressure (SBP) and total score of awareness was -0.352Pu 0.000g, and the correlation coefficient between DBP and total score of awareness was -0.262g / kg. Conclusion the detail management of hypertension in Shanghai community is an effective measure to improve the knowledge and blood pressure control level of hypertension patients.
