基于ALFF的OSAHS患者腦功能變化及臨床相關(guān)研究

【摘要】:目的:探索阻塞性睡眠呼吸暫停低通氣綜合征(obstructive sleep apnea-hypopnea syndrome,OSAHS)患者和正常對照組的大腦神經(jīng)元自發(fā)活動差異及其與OSAHS嚴重程度的關(guān)系。方法:收集30例OSAHS患者及體檢健康者23例,均接受靜息態(tài)功能磁共振(resting-state functional magnetic resonance imaging,rs-fMRI)掃描及MoCA評分,患者組進行多導睡眠監(jiān)測,并在磁共振掃描前24h內(nèi)進行了上氣道CT測量,對兩組圖像數(shù)據(jù)進行處理比較并進行相關(guān)性分析。結(jié)果:與HC組相比,OSAHS組雙側(cè)海馬旁回、顳葉及梭狀回、右側(cè)額葉及左側(cè)小腦低頻振幅活動明顯增強,差異具有統(tǒng)計學意義(P0.005);雙側(cè)頂葉、兩側(cè)楔前葉及前扣帶回的低頻振幅活動減弱,差異具有統(tǒng)計學意義(P0.005);經(jīng)Pearson相關(guān)性分析結(jié)果顯示,雙側(cè)海馬旁回、顳葉及梭狀回與OSAHS組ALFF呈負相關(guān),雙側(cè)頂葉ALFF值與OSAHS患者的呼吸暫停低通氣指數(shù)(apnea-hypopnea index,AHI)呈顯著負相關(guān)(P0.005);雙側(cè)楔前葉及前扣帶回的ALFF值與MoCA評分之間呈顯著正相關(guān)(P0.005)。結(jié)論:fMRI技術(shù)能夠探測到OSAHS患者腦組織功能發(fā)生的變化,為OSAHS患者和正常對照者的鑒別與診斷提供有效的影像學方法。
[Abstract]:Objective: to explore obstructive sleep apnea-hypopnea syndrome in obstructive sleep apnea hypopnea syndrome. The relationship between the spontaneous activity of brain neurons and the severity of OSAHS in patients with OSAHS and normal controls. Methods: 30 patients with OSAHS and 23 healthy controls were collected. All patients received rest state functional magnetic resonance imaging. Rs-fMRI (rs-fMRI) scan and MoCA score, polysomnography was performed in the patient group, and the upper airway CT was measured within 24 hours before Mr scan. Results: compared with HC group, OSAHS group had bilateral parahippocampal gyrus, temporal lobe and fusiform gyrus. The low frequency amplitude activity in the right frontal lobe and left cerebellum was significantly increased (P 0.005). In bilateral parietal lobe, the low frequency amplitude activity of bilateral anterior cuneate lobe and anterior cingulate gyrus decreased, and the difference was statistically significant (P 0.005). The results of Pearson correlation analysis showed that bilateral parahippocampal gyrus, temporal lobe and fusiform gyrus were negatively correlated with ALFF in OSAHS group. Bilateral parietal lobe ALFF was negatively correlated with apnea hypopnea index (P0.005) in patients with OSAHS. There was a significant positive correlation between ALFF and MoCA score in bilateral anterior wedge and anterior cingulate gyrus (P0.005). Conclusion the brain tissue function of patients with OSAHS can be detected by the technique of FMRI. To provide an effective imaging method for the differentiation and diagnosis of OSAHS patients and normal controls.
