【摘要】：研究目的通過探討圍絕經(jīng)期女性認知因素、心理因素、治療概況與生活習慣因素中可能相關的危險因素與圍絕經(jīng)期發(fā)生復發(fā)性尿路感染的關系,以及通過采集圍絕經(jīng)期女性的中醫(yī)四診信息,初探圍絕經(jīng)期復發(fā)性尿路感染的中醫(yī)證候特點,以期全面認識圍絕經(jīng)期復發(fā)性尿路感染患者在認知、心理狀況、治療概況、生活習慣和中醫(yī)證候特點等方面中與疾病發(fā)生相關的危險因素,為臨床工作中指導圍絕經(jīng)期復發(fā)性尿路感染的預防及提出健康宣教方案提供可靠的依據(jù),通過糾正圍絕經(jīng)期女性對疾病的發(fā)生及治療中出現(xiàn)的認識、行為誤區(qū)和不良生活習慣,降低疾病發(fā)生的感染率與復發(fā)率,從而提高圍絕經(jīng)期女性的生活質量;同時通過初步探討圍絕經(jīng)期女性復發(fā)性尿路感染的中醫(yī)證候特點,為圍絕經(jīng)期復發(fā)性尿路感染疾病的中醫(yī)藥防治方案墊定理論基礎。研究方法本研究為非配對病例對照研究,采用便利抽樣的方法,收集2016年4月1日至2017年1月30日就診于北京中醫(yī)藥大學第三附屬醫(yī)院門診及體檢中心、北京市朝陽區(qū)小關社區(qū)、北京市東風社區(qū)衛(wèi)生服務中心已確診或既往確診過為復發(fā)性尿路感染且符合納入標準的圍絕經(jīng)期女性30例,作為病例組(rUTI組),同期在以上醫(yī)院或社區(qū)就診的非復發(fā)性尿路感染的圍絕經(jīng)期女性30例,作為對照組(非rUTI組);通過調查問卷表收集圍絕經(jīng)期女性對復發(fā)性尿路感染疾病的認知、心理、治療概況資料、生活習慣資料以及中醫(yī)四診資料。統(tǒng)一使用SPSS21.0軟件對認知、心理、治療概況資料、生活習慣資料進行統(tǒng)計描述、單因素及多因素非條件Logistic回歸分析,對中醫(yī)四診資料進行頻次描述和系統(tǒng)聚類分析。研究結果通過對認知、心理、治療概況資料、生活習慣資料進行統(tǒng)計描述、單因素分析及多因素非條件logistic回歸分析,本研究最終得出"拖延不就醫(yī)"、"久坐不動"與"憋尿行為"這三個變量與圍絕經(jīng)期復發(fā)性尿路感染的發(fā)生存在正相關,且OR1,表明有拖延不就醫(yī)行為、有久坐不動行為和有憋尿行為均能增加圍絕經(jīng)期復發(fā)性尿路感染發(fā)生的風險。通過對中醫(yī)四診資料進行頻次描述和系統(tǒng)聚類分析,最終得出的結果為:兩組圍絕經(jīng)期女性的中醫(yī)證候均以虛實夾雜證多見,肝腎虧虛為根本;但對照組邪實證的存在相對病例組較少,病例組以痰濕、濕熱、血瘀、肝陽上亢、氣郁等多種邪實證的存在較多。研究結論圍絕經(jīng)期女性存在復發(fā)性尿路感染的危險因素,拖延不就醫(yī)、久坐不動、有憋尿行為均為圍絕經(jīng)期復發(fā)性尿路感染的危險因素。提倡及時就醫(yī),經(jīng)常運動、避免久坐,提倡及時排小便,養(yǎng)成良好的生活習慣,可以降低圍絕經(jīng)期復發(fā)性尿路感染的風險。在中醫(yī)證候特點認識上,圍絕經(jīng)期女性均有肝腎正氣虛損的表現(xiàn),但對比非復發(fā)性尿路感染的圍絕經(jīng)期女性表現(xiàn)為較單純的正氣虧損的證候特點而言,圍絕經(jīng)期復發(fā)性尿路感染女性則在肝腎虧虛的證候基礎上,多兼夾痰濕、濕熱、血瘀、陽亢、氣郁等多種邪實證,因此我們認為,多種邪實證的存在是圍絕經(jīng)期復發(fā)性尿路感染的危險因素,在中醫(yī)防治上,除補益肝腎,培元固本的同時,兼顧祛邪可降低圍絕經(jīng)期復發(fā)性尿路感染的風險。
[Abstract]:Objective to explore perimenopausal women's cognitive factors, psychological factors, and risk factors of perimenopausal relationship between the occurrence of recurrent urinary tract infections may treat situation and lifestyle factors, and through the acquisition of perimenopausal women in the information of four diagnostic methods of TCM syndrome characteristics of perimenopausal recurrent urinary tract infection, in order to a comprehensive understanding of perimenopausal recurrent urinary tract infection in patients with cognitive, psychological status, treatment of the risk factors associated with disease living habits and characteristics of TCM syndrome and so on, for clinical work to guide the prevention of perimenopausal recurrent urinary tract infection and the health education programs and provide a reliable basis, through the understanding of appearance the occurrence and treatment of perimenopausal women to correct diseases, misunderstandings and bad habits, reduce the rate of infection and recurrence of disease incidence, from To improve perimenopausal women's quality of life; at the same time, by discussing the characteristics of TCM Syndromes of perimenopausal women with recurrent urinary tract infection, the theoretical foundation for TCM Prevention and treatment of perimenopausal recurrent urinary tract infection pad. The research methods for non paired case control study, by convenient sampling. From April 1, 2016 to January 30, 2017 in the Third Affiliated Hospital of Beijing University of Chinese Medicine and Beijing City outpatient medical center, Chaoyang District Xiaoguan community, Dongfeng community health service center in Beijing city had been diagnosed or previous diagnosed for recurrent urinary tract infection and met the inclusion criteria of perimenopausal women in 30 cases, as the case group (rUTI group), over the same period in the hospital or the community treatment of non recurrent urinary tract infection in perimenopausal women in 30 cases, as the control group (non rUTI group); through the questionnaire to collect the perimenopausal Menstruating women on disease of recurrent urinary tract infection in cognition, psychological treatment, profile, lifestyle and TCM diagnostic data. Using SPSS21.0 software for unified cognition, psychological treatment, survey data, statistical description of life habits, non conditional Logistic regression analysis of single factor and multi factor analysis, frequency description and system clustering the diagnostic information of traditional Chinese medicine. Empirical research conclusions of perimenopausal women are risk factors of recurrent urinary tract infection, delayed treatment, sedentary behavior has bieniao were risk factors of perimenopausal recurrent urinary tract infection. To promote timely medical treatment, often exercise, avoid sedentary, advocate timely urination, develop good habits, can reduce the risk of perimenopausal recurrent urinary tract infection. In the understanding of the characteristics of TCM syndrome, perimenopausal women have liver and kidney deficiency of vital qi's performance, but the contrast Perimenopausal women showed non recurrent urinary tract infection as the syndrome characteristics than the pure loss of righteousness, perimenopausal women of recurrent urinary tract infections in the liver and kidney deficiency syndrome on the basis of many with phlegm dampness, damp heat, blood stasis, hyperactivity of Yang, Qi and other evilempirical, because of this we believe that the existence of a variety of evilempirical are risk factors of perimenopausal recurrent urinary tract infection, in traditional Chinese medicine on the prevention and treatment of the liver and kidney tonic, Peiyuan at the same time, taking into account Quxie may reduce the risk of perimenopausal recurrent urinary tract infection.
