【摘要】：目的:為臨床合用5-氨基水楊酸(5-ASA)與硫嘌呤類藥物治療炎癥性腸病(IBD)提供參考。方法:通過檢索Pub Med、Web of Science、萬方數(shù)據(jù)庫、維普數(shù)據(jù)庫和中國(guó)知網(wǎng)等相關(guān)專業(yè)數(shù)據(jù)庫十多年來有關(guān)合用5-ASA對(duì)IBD患者應(yīng)用硫嘌呤類藥物的療效和不良反應(yīng)影響的研究文獻(xiàn),整理、歸納和綜述二者合用對(duì)后者個(gè)體化應(yīng)用的影響及其機(jī)制。結(jié)果與結(jié)論:5-ASA和硫嘌呤類藥物是治療IBD的常用藥物,二者合用會(huì)升高后者活性代謝產(chǎn)物濃度,從而增強(qiáng)療效,同時(shí)增大不良反應(yīng)發(fā)生風(fēng)險(xiǎn)。其作用機(jī)制研究主要關(guān)注5-ASA對(duì)硫嘌呤類藥物相關(guān)代謝酶和轉(zhuǎn)運(yùn)體活性的影響以及遺傳因素與上述影響的相關(guān)性,但尚無統(tǒng)一結(jié)論。合用5-ASA對(duì)IBD患者硫嘌呤類藥物個(gè)體化應(yīng)用的影響及其機(jī)制仍有待于多中心、大樣本的前瞻性研究加以證實(shí)。
[Abstract]:Objective: to provide a reference for the treatment of inflammatory bowel disease (IBD) with 5-aminosalicylic acid 5-ASA and thiopurine drugs. Methods: by searching for more than ten years related databases of Pub Med5-ASA, Wanfang database, Weipu database and China Zhiwang database, and other related databases, the therapeutic effect and adverse effects of 5-ASA on the efficacy and adverse reactions of thiopurine drugs in IBD patients were collected and sorted out. The effect and mechanism of the combination of the two on the individualized application of the latter are summarized and summarized. Results: the two drugs are commonly used in the treatment of IBD. The concentration of active metabolites of the latter can be increased by the combination of the two drugs, thus enhancing the efficacy and increasing the risk of adverse reactions. The effect of 5-ASA on thiopurine drug-related metabolic enzymes and transporter activities and the correlation between genetic factors and these effects were studied, but there was no uniform conclusion. The effect of 5-ASA on the individualized application of thiopurine drugs in IBD patients and its mechanism need to be confirmed by a large sample of prospective studies.
