【摘要】：目的觀(guān)察氧化苦參堿(OM)不同途徑給藥治療重癥急性胰腺炎(SAP)的效果。方法 70例SAP患者,按隨機(jī)數(shù)字表法分為四組:對(duì)照組(15例)接受基礎(chǔ)治療,OM灌洗組(15例)在基礎(chǔ)治療同時(shí)接受OM腹腔灌洗治療,OM灌腸組(20例)在基礎(chǔ)治療同時(shí)接受OM灌腸治療,OM靜滴組(20例)在基礎(chǔ)治療同時(shí)接受OM靜脈滴注治療。觀(guān)察各組患者臨床癥狀的變化并于治療開(kāi)始前(D0)和治療后24 h(D1)、48 h(D2)、72 h(D3)檢測(cè)血清內(nèi)毒素和血漿D-乳酸,于D4和D7檢測(cè)血清IL-1、IL-6和TNF-α。結(jié)果 OM靜滴組的腹痛緩解時(shí)間、血清淀粉酶恢復(fù)時(shí)間和住院時(shí)間短于OM灌腸組和OM灌洗組(P均0.05),OM灌腸組的首次排便時(shí)間短于OM靜滴組和OM灌洗組(P均0.05)。D1、D2、D3時(shí),OM靜滴組和OM灌腸組的血清內(nèi)毒素和血漿D-乳酸水平低于OM灌洗組(P均0.05);D4、D7時(shí),OM靜滴組的血清IL-1、IL-6、TNF-α水平低于OM灌腸組和OM灌洗組(P均0.05)。結(jié)論 OM治療SAP的三種給藥途徑相比,靜滴效果最佳且患者接受度高,灌腸給藥可促進(jìn)排便,腹腔灌洗的效果略差。
[Abstract]:Objective to observe the efficacy of oxymatrine (OM) in the treatment of severe acute pancreatitis (SAP). Methods 70 patients with SAP were randomly divided into four groups: the control group (15 cases) received basic treatment and the OM lavage group (15 cases) received OM peritoneal lavage at the same time. OM enema group (n = 20) received OM enema at the same time as basic therapy and OM intravenous drip group (n = 20) received OM intravenous drip at the same time. The changes of clinical symptoms were observed and serum endotoxin and plasma D-lactic acid were detected at 24 h (D1), 48 h (D2), 72 h (D3) before and after treatment, and serum IL-1, was detected at D4 and D7. IL-6 and TNF- 偽. Results the abdominal pain relief time, serum amylase recovery time and hospitalization time of OM group were shorter than those of OM enema group and OM lavage group (P 0.05). The first defecation time of OM enema group was shorter than that of OM intravenous drip group and OM lavage group (P 0.05). The serum endotoxin and plasma D-lactic acid levels in OM intravenous drip group and OM enema group were lower than those in OM lavage group (P 0.05). The serum IL-1,IL-6,TNF- 偽 level in OM group was lower than that in OM enema group and OM lavage group (P 0.05). Conclusion compared with the three methods of OM in the treatment of SAP, intravenous drip is the best and the acceptance of patients is high. Enema can promote defecation, and the effect of abdominal lavage is a little poor.
