生育津貼制度研究

【摘要】:生育醫(yī)療費用和生育津貼是生育保障待遇的兩個方面。生育醫(yī)療費用并入社會醫(yī)療保險,是國外大多數(shù)國家的做法:近幾年,我國正積極通過生育保險和醫(yī)療保險協(xié)同推進的方法,解決工薪勞動者生育醫(yī)療費用保障問題,同時在新型農(nóng)村合作醫(yī)療制度和城鎮(zhèn)居民基本醫(yī)療保險制度中解決非工薪勞動者生育醫(yī)療費用保障問題。但是,對于生育津貼,目前僅限于工薪勞動者,廣大非就業(yè)育齡女性群體被排除在制度之外,而且生育津貼計發(fā)標準、支付方式各地差異較大:人群分等、制度分設、待遇懸殊的生育保障制度安排.直接影響著社會公平和制度運行效率。 本研究從中國實際出發(fā),借鑒國際經(jīng)驗,對我國生育保障制度進行規(guī)劃和設計,提出將生育醫(yī)療費用納入社會醫(yī)療保險,單獨設置覆蓋全民的生育津貼制度。對于生育津貼制度,通過建立精算模型,對制度覆蓋水平下的津貼領取人數(shù)、用人單位繳費率和資金需求量進行測算。測算結果表明,我國未來十五年非就業(yè)育齡女性人數(shù)幾近就業(yè)育齡女性人數(shù)的兩倍,人數(shù)龐大的非就業(yè)女性恰恰是目前的生育保險制度忽略的群體:用人單位繳費率不超過0.2%,遠遠低于目前的政策要求:生育津貼制度資金需求量占財政收人的比重維持在較低水平.財政支付能力處于完全可以承受的范圍之內(nèi),故而本研究認為,我國現(xiàn)階段完全有能力構建生育津貼制度。生育津貼制度可以有效彌補現(xiàn)行制度的缺失,應該成為我國適度普惠型社會福利體系框架下的一個重要子項制度:為使這一制度能夠有效實施.本研究提出用人單位與政府共同承擔生育津貼制度籌資責任,而生育津貼待遇則應以;緸樵瓌t,待遇標準應該適度。這項研究和有關建議,希望能夠為我國建立更加合理的生育保障制度提供參考建議。
[Abstract]:The birth medical expenses and birth allowance are the two aspects of the birth guarantee treatment. The birth medical cost is incorporated into the social medical insurance, which is the practice of most foreign countries. In recent years, our country is actively using the method of promoting the birth insurance and medical insurance to solve the problem of the maternity cost guarantee of the working workers, and at the same time in the new type of agriculture. The village cooperative medical system and the urban residents' basic medical insurance system solve the problem of medical cost guarantee for non worker workers. However, the birth allowance is only limited to working workers, the majority of non employment women of childbearing age are excluded from the system, and the birth allowance is issued by the standard, and the different ways of payment vary widely. The arrangement of the birth insurance system, which is divided into groups, separated by system, and with a wide range of treatment, directly affects the social equity and the efficiency of the system operation.
Starting from the actual situation of China and drawing on the international experience, this study plans and designs the system of maternity protection in China, puts forward the inclusion of childbearing medical expenses into the social medical insurance, and sets up a system of childbearing allowance to cover the whole people alone. The calculation results show that the number of non employment women in the next fifteen years is nearly two times the number of women of childbearing age, and the large number of non employment women is just the group neglected by the current childbearing insurance system: the payment rate of employers is not more than 0.2%, far below the current policy. It is required: the fund demand of the birth allowance system is maintained at a low level of the proportion of the financial recipients. The financial payment ability is within the full scope. Therefore, the present study believes that the birth allowance system is fully capable of building the birth allowance system at the present stage. The birth allowance system can make up for the lack of the current system and should be our country. An important subitem system under the framework of the moderate and inclusive social welfare system: to enable the system to be implemented effectively. This study suggests that the employer and the government jointly undertake the responsibility for raising funds for the birth allowance system, while the birth allowance treatment should be based on the basic principle and the treatment standard should be moderate. It can provide a reference for our country to establish a more reasonable birth security system.
