【摘要】：目的總結(jié)幾種少見病理亞型腎細(xì)胞癌的動(dòng)態(tài)增強(qiáng)CT表現(xiàn),提高影像診斷水平。方法結(jié)合相關(guān)文獻(xiàn),回顧性分析經(jīng)手術(shù)病理證實(shí)的11例少見病理亞型腎癌患者動(dòng)態(tài)增強(qiáng)CT表現(xiàn)。結(jié)果 5例乳頭狀細(xì)胞癌,4例平掃密度不均,1例為囊性低密度影,增強(qiáng)掃描呈少血供表現(xiàn);4例嫌色細(xì)胞癌,3例平掃密度均勻,1例密度不均,增強(qiáng)掃描呈均勻或不均勻輕中度強(qiáng)化;1例Bellini集合管癌,平掃表現(xiàn)為腎臟體積增大,密度不均,腫瘤界限不清,增強(qiáng)掃描呈輕度不均勻延時(shí)強(qiáng)化,并侵犯腎周脂肪囊;1例Xp11.2易位/TFE3基因融合相關(guān)性腎癌,平掃病灶內(nèi)見多發(fā)環(huán)形鈣化灶,增強(qiáng)掃描病灶實(shí)性部分明顯強(qiáng)化。結(jié)論 CT對(duì)少見病理亞型腎癌的定性診斷有一定的價(jià)值,但確診還應(yīng)依賴于病理學(xué)檢查。
[Abstract]:Objective to summarize the dynamic enhanced CT findings of several rare pathological subtypes of renal cell carcinoma (RCC) and to improve the imaging diagnosis. Methods the dynamic enhanced CT findings of 11 patients with rare pathological subtypes of renal cell carcinoma confirmed by surgery and pathology were analyzed retrospectively. Results in 5 cases of papillary cell carcinoma, 4 cases of papillary cell carcinoma showed uneven density on plain scan, 1 case showed cystic low density, and 4 cases showed hypoglycemia on enhanced scan. 3 cases of chromophobe cell carcinoma showed homogeneous and even density in 1 case. One case of Bellini collecting duct carcinoma showed homogeneous or uneven enhancement on contrast enhanced scan. The plain scan showed that the renal volume was enlarged, the density was uneven, the tumor boundary was not clear, and the enhancement scan was slightly uneven and delayed enhancement. One case of Xp11.2 translocation / TFE3 gene fusion associated with renal cell carcinoma was involved in perirenal fat cysts. Multiple circular calcification foci were found in the lesions on plain scan. Conclusion CT is valuable for the qualitative diagnosis of rare renal cell carcinoma, but the diagnosis should depend on pathological examination.
