【摘要】：目的探討烏司他丁(UTI)對(duì)重癥急性胰腺炎(SAP)大鼠腸黏膜屏障的影響及其作用機(jī)制。方法 SD大鼠60只分成對(duì)照組、假手術(shù)(SO)組、SAP組和UTI組。制模后每隔8 h進(jìn)行腹腔內(nèi)注射UTI(濃度1.5萬(wàn)IU/kg)作為UTI組。各治療組檢測(cè)制模后8、24、72 h血漿內(nèi)毒素水平,通過(guò)RT-PCR法和免疫組化法對(duì)腸黏膜Toll樣受體(TLR)4表達(dá)進(jìn)行檢測(cè),并在電鏡下觀察腸黏膜組織。結(jié)果對(duì)照組腸黏膜未見(jiàn)TLR4表達(dá)。與對(duì)照組相比,SAP組在制模后8 h血漿內(nèi)毒素和腸黏膜TLR4表達(dá)即出現(xiàn)升高,并隨時(shí)間的延長(zhǎng)而繼續(xù)進(jìn)行性增高。血漿內(nèi)毒素與腸黏膜TLR4二者明顯相關(guān)。UTI組同時(shí)點(diǎn)內(nèi)毒素、TLR4低于SAP組,電鏡下檢查發(fā)現(xiàn)病理?yè)p傷也同時(shí)減輕,24 h時(shí)最為明顯。結(jié)論 UTI能有效保護(hù)SAP發(fā)生時(shí)的腸屏障功能,其保護(hù)機(jī)制可能與腸黏膜TLR4的表達(dá)下調(diào)有關(guān)。
[Abstract]:Objective to investigate the effect and mechanism of ulinastatin (UTI) on intestinal mucosal barrier in rats with severe acute pancreatitis (SAP). Methods 60 SD rats were divided into control group, sham operation group and UTI group. The UTI group was treated by intraperitoneal injection of UTI (15000 IU / kg) every 8 hours. The plasma endotoxin level was measured at 824 hours after model making in each treatment group. The expression of Toll like receptor TLR4 in intestinal mucosa was detected by RT-PCR and immunohistochemistry, and the intestinal mucosal tissue was observed under electron microscope. Results there was no TLR4 expression in intestinal mucosa of control group. Compared with the control group, plasma endotoxin and intestinal mucosal TLR4 expression increased at 8 h after model making, and continued to increase progressively with the prolongation of time. There was a significant correlation between plasma endotoxin and intestinal mucosal TLR4. TLR4 in UTI group was lower than that in SAP group at the same time. Conclusion UTI can effectively protect the intestinal barrier function during the occurrence of SAP, and its protective mechanism may be related to the down-regulation of TLR4 expression in intestinal mucosa.
