輕微肝性腦病患者默認模式網(wǎng)絡(luò)功能連接研究

【摘要】:目的采用靜息態(tài)功能磁共振成像(rs-fMRI)研究輕微肝性腦病患者默認網(wǎng)絡(luò)(DMN)腦區(qū)功能連接的改變情況。方法本研究共納入73例肝硬化患者和73名匹配的健康對照者進行rs-fMRI掃描,采用DPARSF軟件分析得到DMN相關(guān)腦區(qū)功能連接的變化情況,并使用方差分析比較各組之間差異;使用皮爾森相關(guān)分析DMN中腦區(qū)功能連接改變和神經(jīng)心理功能測試結(jié)果之間的聯(lián)系,ROC分析評估可靠性。結(jié)果與HC組相比,肝硬化患者DMN中額前內(nèi)側(cè)回(MPFC),后扣帶回/楔前葉(PCC/PCUN),左側(cè)顳極(TP)以及雙側(cè)海馬旁回(PHG)DMN連接下降,而與non-HE患者相比,MHE組患者在MPFC,PCC/PCUN,以及右側(cè)PHG腦區(qū)間的功能連接降低。其中MPFC出現(xiàn)改變的有效性最高(敏感度81.5%,特異度70.4%)。結(jié)論肝硬化患者從non-HE發(fā)展為MHE的過程中,DMN部分腦區(qū)的功能連接減低,這為輕微肝性腦病的診斷提供了一種新的思路。
[Abstract]:Objective to study the changes of functional connections in the default network of (DMN) in patients with mild hepatic encephalopathy by resting functional magnetic resonance imaging (rs-fMRI). Methods 73 patients with liver cirrhosis and 73 matched healthy controls were included in this study. The changes of functional connections in DMN related brain regions were analyzed by DPARSF software and the differences between the groups were compared by ANOVA. Pearson correlation was used to analyze the relationship between the changes of functional connections in the midbrain area of DMN and the results of neuropsychological function tests. ROC analysis was used to evaluate the reliability. Results compared with the HC group, the posterior cingulate gyrus (PCC/PCUN), the left temporal pole (TP) and the (PHG) DMN junction in the bilateral parahippocampal gyrus were decreased in DMN patients with DMN, but compared with those in non-HE patients. The functional junctions in MPFC,PCC/PCUN, and right PHG were decreased in MHE group. Changes in MPFC were most effective (sensitivity 81.5%, specificity 70.4%). Conclusion in the process of developing from non-HE to MHE, the functional junctions of some areas of DMN in patients with liver cirrhosis are decreased, which provides a new way for the diagnosis of mild hepatic encephalopathy.
