【摘要】：目的:檢驗"多繳多得"政策對參保的經(jīng)濟(jì)激勵效果。方法:基于參保人理性選擇視角,建立個人繳費及賠付的精算現(xiàn)值模型,借助補償收益率指標(biāo),測算不同繳費檔次模式下的成本收益情況。結(jié)果:在醫(yī)療消費支出高的年齡參保、低檔次繳費是理性選擇,該政策起到了相反的經(jīng)濟(jì)激勵效果,即多繳并未多得。結(jié)論:"多繳多得"的政策設(shè)計違背了基本醫(yī)療保險的公平性原則,不利于制度的良好穩(wěn)定發(fā)展及社會公平,應(yīng)當(dāng)從完善長效籌資機制,實行強制參保入手,消除參保過程中的逆向選擇。
[Abstract]:Objective: to test the economic incentive effect of the policy. Methods: based on the rational choice of the insured, an actuarial present value model of individual payment and compensation was established, and the cost and income of different payment levels were calculated by means of the compensation rate index. Results: in the age of high medical expense, the low grade payment is the rational choice. This policy has the opposite economic incentive effect, that is, the overpayment is not too much. Conclusion: the policy design of "paying more and getting more" violates the principle of fairness of basic medical insurance, and is not conducive to the good and stable development of the system and social fairness. Therefore, we should start with perfecting the long-term financing mechanism and implementing compulsory insurance. Eliminate adverse selection in the process of insurance participation.
