【摘要】：目的:探討我院急性上消化道出血的病因分布、比較不同處理措施的治療效果,為我院進(jìn)一步規(guī)范上消化道出血的診療提供較可靠的臨床依據(jù)。方法:回顧性分析2013年2月至2016年2月我院急診科收治的189例急性上消化道出血病人的詳細(xì)的臨床資料。結(jié)果:189例急性上消化道出血的患者中消化性潰瘍108例,其中十二指腸潰瘍56例,胃潰瘍52例,食管胃底靜脈曲張破裂36例,急性胃粘膜病變20例,胃癌16例,其他病因9例。其中藥物治療好轉(zhuǎn)或治愈106例,胃鏡下治療好轉(zhuǎn)或治愈45例,介入治療好轉(zhuǎn)或治愈14例,轉(zhuǎn)外科手術(shù)治療好轉(zhuǎn)或治愈7例,死亡總?cè)藬?shù)17例。行胃鏡檢查者156例。結(jié)論:我院急診科收治的急性上消化道出血的病人病因以消化道潰瘍?yōu)橹?胃鏡有很好的診斷價(jià)值,治療上現(xiàn)仍以內(nèi)科藥物治療為主。上消化道出血一般發(fā)病較急,病情進(jìn)展較快,入院后需迅速評(píng)估病情,積極給予對(duì)癥處理維持生命體征平穩(wěn),同時(shí)盡快完善胃鏡明確病因,制定后續(xù)治療方案,降低死亡率。
[Abstract]:Objective: To investigate the etiology of acute upper gastrointestinal hemorrhage, comparison of different treatment measures in our hospital for treatment, to further standardize the diagnosis and treatment of upper gastrointestinal bleeding and provide clinical evidence is reliable. Methods: To review the detailed clinical data of 189 patients with acute upper digestive tract emergency department of our hospital from February 2013 to February 2016 analysis treatment of hemorrhage. Results: 189 cases of acute upper gastrointestinal bleeding in patients with peptic ulcer in 108 cases, including 56 cases of duodenal ulcer, 52 cases of gastric ulcer, esophageal and gastric varices in 36 cases, 20 cases of acute gastric mucosal lesions, 16 cases of gastric cancer, 9 cases of other causes. The medical treatment or endoscopic treatment of 106 cases were cured, 45 cases improved or cured, 14 cases improved or cured interventional therapy, surgical operation treatment 7 cases improved or cured, the total number of deaths in 17 cases. 156 cases underwent gastroscopy. Conclusion: admitted to the emergency department of our hospital Acute upper gastrointestinal bleeding in patients with peptic ulcer disease, endoscopy is valuable in diagnosis and treatment is still in medical treatment. Upper gastrointestinal bleeding is generally more acute onset, disease progressed quickly, quickly assess the condition required after admission, positive symptomatic treatment to maintain stable vital signs, and perfect as soon as possible gastroscope clear etiology, follow-up treatment plan, formulated to reduce the mortality rate.
