【摘要】:醫(yī)療聯(lián)合體在世界范圍上獲得的重要成就和成功經(jīng)驗表明,醫(yī)療聯(lián)合體的建立和發(fā)展有利于深化醫(yī)藥衛(wèi)生體制改革,組建醫(yī)療聯(lián)合體,有效地發(fā)揮三級三甲醫(yī)院的專業(yè)技術優(yōu)勢及帶頭作用,構(gòu)建急慢分治、分級診療、雙向轉(zhuǎn)診的診療模式,促進醫(yī)療機構(gòu)相互間的分工與合作,合理配置醫(yī)療衛(wèi)生資源。安徽ET醫(yī)療聯(lián)合體在這種浪潮中應運而生,在安徽省內(nèi)發(fā)展迅速,逐漸成為開展醫(yī)護業(yè)務培訓、促進分級診療,推進雙向轉(zhuǎn)診等的重要平臺。安徽省ET醫(yī)療聯(lián)合體的建立和發(fā)展,受到了本地政府、衛(wèi)生部門以及學者的廣泛關注。資源依賴理論的觀點認為,組織的存在和發(fā)展離不開與外部資源的交換,在組織生存和發(fā)展的過程中,需要依賴外部的資源,在眾多的資源中,識別出依賴資源,并且獲取該資源的能力對于組織的發(fā)展是至關重要的。本研究通過借鑒資源依賴理論的基本觀點,對安徽省ET醫(yī)療聯(lián)合體從建立之初到發(fā)展至今這一歷程進行深入分析,調(diào)研過程中,采取了文獻研究法、參與觀察、個案訪談與開小型座談會四種形式相結(jié)合的研究方法收集資料,通過理論分析與實地調(diào)研,對安徽省ET醫(yī)療聯(lián)合體發(fā)展情況,影響醫(yī)療聯(lián)合體的發(fā)展因素,發(fā)展過程中遇到的問題進行分析,提出解決問題的措施。具體來說本文由以下部分組成:第一章為導論部分,介紹醫(yī)療聯(lián)合體發(fā)展的背景及研究的意義,總結(jié)其他學者對醫(yī)療聯(lián)合體的研究成果,界定相關概念,對資源依賴理論進行分析和闡述。第二章為研究設計。根據(jù)本文的研究對象——安徽省ET醫(yī)療聯(lián)合體,收集資料,分析研究問題,制定可行的方案。第三章分析安徽省ET醫(yī)療聯(lián)合體的現(xiàn)狀,主要介紹了安徽省ET醫(yī)療聯(lián)合體的建立和發(fā)展及影響安徽省ET醫(yī)療聯(lián)合體發(fā)展的主要因素,即安徽省ET醫(yī)療聯(lián)合體依賴的環(huán)境資源,符號資源,維持醫(yī)療聯(lián)合體基本活動的資源,權力合法性資源四種資源,安徽省ET醫(yī)療聯(lián)合體的主要依賴對象為安徽省衛(wèi)計委和安徽省ET醫(yī)院及北京ET醫(yī)療集團。第四章主要闡述安徽ET醫(yī)療聯(lián)合體的發(fā)展困境,分別從內(nèi)部和外部原因進行分析,得出內(nèi)部主要困境為:組織結(jié)構(gòu)不緊密,利益分配制度不完善、組織成員積極性不高;外部主要困境為:體制機制障礙、文化觀念阻礙、信息化建設滯后、現(xiàn)有制度不健全。第五章提出安徽省ET醫(yī)療聯(lián)合體走出發(fā)展困境的措施,分別從組織內(nèi)部和外部提出建議,主要包括建立緊密型組織結(jié)構(gòu),合理分配利益,提高成員單位的組織認同感,消除機制障礙,提升基層服務能力,主管部門完善醫(yī)療聯(lián)合體的相關配套政策,發(fā)揮醫(yī)保杠桿作用,按病種制定醫(yī)療聯(lián)合體轉(zhuǎn)診、分級診療的標準,加快信息化及遠程醫(yī)療平臺的建設,注重文化引導,有序推進工作等措施。
[Abstract]:The important achievements and successful experiences of the medical association in the world show that the establishment and development of the medical association is conducive to deepening the reform of the medical and health system and establishing the medical association. In order to promote the division of labor and cooperation among medical institutions, we should give full play to the professional and technical advantages and the leading role of the third Class A Hospital, construct the diagnosis and treatment mode of acute and slow division of treatment, classified diagnosis and treatment, two-way referral, and promote the division of labor and cooperation among medical institutions. Rational allocation of medical and health resources. Anhui et medical association emerged as the times require in this wave, rapid development in Anhui Province, gradually become to carry out medical training, promote grading diagnosis and treatment. The establishment and development of et medical complex in Anhui Province has been widely concerned by local government, health department and scholars. The existence and development of an organization can not be separated from the exchange of external resources. In the process of organization's survival and development, it needs to rely on external resources, and among many resources, it can be identified as dependent resources. And the ability to obtain the resource is very important for the development of the organization. From the beginning of the establishment to the development of et medical association in Anhui Province from the beginning to the development of this process in-depth analysis, in the process of investigation, the use of literature research method, to participate in the observation. Through theoretical analysis and field investigation, this paper analyzes the development of et medical association in Anhui Province, and influences the development factors of the medical association. This paper is composed of the following parts: the first chapter is the introduction part, introduces the background of the development of the medical association and the significance of the research. Summarize the research results of other scholars on the medical association, define the relevant concepts. The second chapter is the research design. According to the research object of this paper, the et medical association in Anhui Province, collect the data and analyze the research problems. The third chapter analyzes the current situation of et medical association in Anhui Province, mainly introduces the establishment and development of et medical association in Anhui Province and the main factors affecting the development of et medical association in Anhui Province. There are four kinds of resources: environmental resources, symbol resources, basic activities resources and power legitimacy resources of et medical association in Anhui province. The main dependent objects of et medical association in Anhui Province are Anhui Health and Family Planning Commission, et Hospital of Anhui Province and et Medical Group of Beijing. Chapter 4th mainly expounds the development dilemma of et medical association in Anhui Province. From the analysis of internal and external reasons, it is concluded that the main internal difficulties are: the organizational structure is not tight, the benefit distribution system is not perfect, and the enthusiasm of the members of the organization is not high; The main external difficulties are: system and mechanism obstacles, cultural concept obstacles, information construction lags behind, the existing system is not perfect. Chapter 5th puts forward the measures to get out of the development dilemma of et medical association in Anhui Province. Respectively from the organization internal and external suggestions, including the establishment of a tight organizational structure, reasonable distribution of benefits, improve the organizational identity of member units, remove mechanism barriers, improve grass-roots service capacity. The competent departments improve the relevant supporting policies of the medical association, give play to the role of leverage of medical insurance, according to the type of disease to formulate medical association referral, classification of diagnosis and treatment standards, accelerate the construction of information and telemedicine platform. Pay attention to cultural guidance, orderly work and other measures.
