摘要：

目的：分析七氟醚和氨氟醚低流量麻醉在臨床麻醉應(yīng)用中的效果比較。方法選取2010年3月～2013年3月在湘鄉(xiāng)市人民醫(yī)院行美國麻醉醫(yī)師協(xié)會(huì)（ASA）Ⅰ～Ⅱ級擇期腹部外科手術(shù)患者124例，隨機(jī)均分為七氟醚組和氨氟醚組（n=62）。所有患者經(jīng)麻醉誘導(dǎo)后行氣管插管，再使患者與麻醉機(jī)相連。七氟醚組吸入七氟醚來麻醉維持，氣體流速為0.8～1.0 L/min，氨氟醚組吸入氨氟醚來麻醉維持，氣體流速為0.3～0.5 L/min，麻醉維持過程中保持1.9%的七氟醚濃度和1.7%的氨氟醚濃度。記錄患者拔管時(shí)間、睜眼時(shí)間、正確回答姓名時(shí)間、正確回答生日時(shí)間、發(fā)生躁動(dòng)的例數(shù)、發(fā)生惡心嘔吐的例數(shù)。結(jié)果七氟醚組患者的睜眼時(shí)間與氨氟醚組比較，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）；其中，七氟醚組合安氟醚組的拔管時(shí)間（min）、睜眼時(shí)間（min）、回答姓名時(shí)間（min）和回答生日時(shí)間（min）分別為：（14.67±3.85）、（6.72±3.21）、（18.32±3.62）和（19.07±8.86）以及（15.54±4.08）、（20.86±5.37）（、19.89±6.85）和（21.64±8.75）。七氟醚組患者的躁動(dòng)發(fā)生率、惡心嘔吐發(fā)生率（9.68%和4.84%）與氨氟醚組（25.81%和20.97%）比較，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。結(jié)論七氟醚的麻醉效果優(yōu)于氨氟醚，具有麻醉誘導(dǎo)快、對呼吸道刺激作用小、蘇醒時(shí)間短、不良反應(yīng)發(fā)生率低等特點(diǎn)。

Abstract： 

Objective To compare the side effects of sevolfurane and enlfurane in low-lfow anesthesia in clinical applications. Methods 124 patients were selected who carried ASAⅠtoⅡgrade abdominal surgery in the hospital from March 2010 to March 2013, randomly divided into sevolfurane group and enlfurane group, 62 cases in each group. All patients underwent endotracheal intubation after induction of anesthesia, and then connected the patient with anesthesia machine. Sevolfurane group inhalation sevolfurane to anesthetic maintain, the gas lfow rate was 0.8-1.0 L/min,enlfurane group inhalation enlfurane to anesthesia maintenance, the gas lfow rate was 0.3-0.5 L/min,anesthesia maintenance process to maintain 1.9%sevolfurane concentration and 1.7%enlfurane concentration. Recorded extubation time, eye opening time, the time of answer name correct, the time of answer birthday correct, the number of occurring restlessness cases, the number of occurring nausea and vomiting cases. Results Time to eye opening in sevolfurane group and enlfurane group had statistically signiifcant difference (P<0.05);Sevolfurane combination of enlfurane group of decannulation time (min), open time (min), answer name time (min) and answer the birthday time (min), respectively:14.67±3.85, 6.72±3.21, 18.32±3.62 and 19.07±8.86 and 15.54±4.08, 20.86±5.37 and 19.89±6.85 and 21.64±8.75. Sevolfurane group of patients with agitation rate, incidence of nausea and vomiting (9.68%and 4.84%) and ammonia deslfurane group (25.81%and 20.97%). Conclusion Sevolfurane anesthesia effect is better than enlfurane,it has the characteristics of fast induction of anesthesia, respiratory tract irritation small, short recovery time and low incidence of adverse reactions.
